The computation of your package is set to Dish Network America's Top 200 with Local Channels and Free HD for Life Package. I also included 1 Standard Receiver and 1 HD Receiver in the computation. On your first 12-months of Pay-TV service, this should be your monthly billing statement:

1 STD and 1 HD - $0.00
America's Top 200 - $39.99
Locals, AT200+HD - $0.00
Total - $39.99
